Physical Forms & Liability Waivers

Please download the full Athletic Physical Form packet. The COVID-19 Waiver as well as the Insurance Form are included in the link below. It is mandatory to complete it before tryouts begin. Student-Athletes will not be allowed to practice or participate in any physical activity without proper documentation.

For the Supplemental Insurance the fee is $20

For the Notary Stamp the fee is $5
